COVID-19 Testing and Registration

Please use the following links to sign up for a COVID-19 test in our parking lot. Do not enter the clinic for COVID-19 testing!  Por favor, use los siguientes enlaces para registrarse para una prueba de COVID-19 en nuestro estacionamiento. ¡No ingrese a la clínica para la prueba de COVID-19!

Remember that both of the below must be completed:

  1. Registration Link: https://register.covidconnect.wi.gov/en-US/
    1. *Registrants should print or save the associated QR code, as it will be used for their registration. If misplaced, their registration can be looked up by name, date of birth, email address, or phone number.
  2. Scheduling Link: https://forms.gle/bLaCvhiN4gFGxDvy8
    1. Registrants should start by determining if they will DRIVE THROUGH or WALK UP for their testing. Then, select a testing date and appointment slot. A confirmation email will be sent to them with the details of their appointment if they enter a valid email address.

CLINIC PROTOCOLS AND PROCEDURES

  • MUST WEAR A MASK AT ALL TIMES. NO EXCEPTIONS.
  • Must sanitize hands upon arrival.
  • Must go through a screening process in order to be seen by a provider. This includes symptom questions and temperature taken upon arrival.
  • Only the patient can be in the lobby, no relatives or friends. Exceptions may be requested.
  • Patients must arrive exactly at appointment time.

Waukesha Free Clinic takes this pandemic very seriously. We are monitoring the situation routinely to provide care to the community accordingly and safely. WFC staff and volunteers consistently clean and sanitize the clinic to ensure the safety of our new and existing patients.

Covid-Gloves

COVID Kits

Patients of WFC who have been diagnosed with COVID-19 or exposed to someone with the diagnosis will receive the appropriate supplies, medication, and support. These COVID kits include gloves, sanitation supplies, medication, one pulse oximeter, and masks. Along with the kits, WFC providers will keep close communication to monitor the virus.
